Reporting Accidents, Near Misses and Hazards
All accidents, near misses and hazards must be reported to Health, Safety & Wellbeing without delay and always within 24 hrs, using one of the methods below.
- Complete an Accident form online via the HR Website Quick Links, on the left menu:
- Alternatively, if you do not have access to a computer - submit an Accident Form to Health, Safety & Wellbeing (forms are available from the Health & Safety Co-ordinator).
If an accident has resulted in serious injury, or a hazard is so serious that it could result in an accident in the immediate future, it is important that these facts are reported immediately by contacting the Health, Safety & Wellbeing Helpline on ext 55425.
Once notified Health, Safety & Wellbeing may wish to examine the circumstances relating to the report and may contact the individuals concerned and/or the Health & Safety Co-ordinator to help with their enquiries.
Recording of Accidents, Near Misses and Hazards
Health, Safety & Wellbeing collate accident and near miss statistics for reporting to the University Health and Safety Advisory Group and Executive Committee.
The Health and Safety Co-ordinator must keep a record of all reported instances occurring in their School or Professional Service.
Definitions for the purpose of this document
- An accident - is an unexpected event, which results in personal harm
- Near miss - is a narrowly avoided accident
- Hazard – is a dangerous situation or object, which has the potential to cause harm
